According to reports from Bend, OR FOX affiliate KTVZ and the Associated Press, an Oregon woman who applied for health coverage through Cover Oregon said the state exchange mailed her the personal information of two other applicants.
Ann Migliaccio told the AP she received information that included the names and birth dates of two other applicants. Migliaccio said the documents did not include Social Security numbers and that she sent the information back to Cover Oregon, who is reportedly investigating the matter.
